Become a COMO Member Application - Individual Affiliate Membership Form

COMO may receive applications from individuals who wish to support the aims of COMO but who are not part of an established organisation as defined inthe Organisation Application section and who do not meet the six criteria necessary for full membership of COMO. Under these circumstances, COMO may accept such applications as affiliate members. Affiliate members will be able to play a full part in COMO and its activities, but will not be eligible to vote on any matters put to a vote of the membership. Admission of Affiliate Members will follow the same process as that for Full Members.

Applications for Affiliate Membership will needs to be made on an individual basis. Applicants will need to provide full details of their reason for wishing to join COMO as an Affiliate Member and explain their work and responsibilities to date and expected future work and commitments with respect to meningitis.

In order for Affiliate members to be admitted, COMO will need to satisfy itself that the individual has sufficient relevant experience and will be able to fulfil the requirements for membership.
