Full Membership Application Form

Organisations must fullfill the six criteria below to become a Full Member:
	The organisation should be registered as a society or charitable organisation within the jurisdiction in which they are based and operate with and/or on behalf of others for the purpose of representing meningitis in all its forms. The organisation must have a legal status appropriate to its country of origin, with a written constitution and/or by-laws. If no appropriate legal status exists in the country of origin this criteria may be waived at the discretion of CoMO.
	The organisation must be non-profit and non-governmental.
	The member organisation may be an international, regional, national or local organisation, or an umbrella group.
	The organisation can demonstrate that it has a governance structure, which shows its commitment to dealing with issues affecting meningitis and/or supporting those affected by the disease.
	The governance structure must be described and illustrated in a written letter or e-mail which should accompany the organisation's application.
	The organisation must demonstrate its commitment to meningitis such as its vision, mission or organisational objectives and activities.
